Care Management Coordinator
About the role
The Care Management Coordinator provides essential clerical support for the Care Management Department. Under the supervision of licensed clinical staff, accountabilities include coordinating communication between patients, health care providers, insurance companies and other stakeholders to ensure safe and efficient transitions of care, payment for clinical services provided and regulatory compliance. The goal is to ensure the maintenance of accurate records, process documentation and support the delivery of quality patient care. This role requires excellent organizational skills, attention to detail, and the ability to handle multiple tasks simultaneously.
Responsibilities
- Coordinate communication between patients, health care providers, insurance companies and other stakeholders
- Ensure safe and efficient transitions of care
- Facilitate payment for clinical services provided
- Maintain regulatory compliance
- Maintain accurate records and process documentation
- Support the delivery of quality patient care
Required Qualifications
- 2 years Administrative experience, healthcare setting preferred or demonstrated knowledge of healthcare terminology

- Associate's Degree in Business, Healthcare, or related field

Pay
Base pay range: $26.37 - $45.52 per hour. Additional compensation may be available such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities. Placement on the wage range determined by relevant job experience and other applicable factors.
