We are seeking an organized, compassionate, and driven Care Coordinator to join our growing team.

Position Summary

The Care Coordinator serves as the primary point of contact for assigned clients and caregivers. This position is responsible for ensuring quality care, maintaining strong client relationships, coordinating services, monitoring caregiver performance, responding to changes in client condition, and working closely with our scheduling, HR, and clinical teams to deliver exceptional service. The ideal candidate thrives in a fast-paced environment, enjoys solving problems, and has a passion for helping others.

Responsibilities

  • Manage an assigned caseload of clients and caregivers.
  • Build and maintain positive relationships with clients, families, caregivers, referral partners, and healthcare professionals.
  • Conduct client welcome calls, routine quality assurance calls, and satisfaction follow-ups.
  • Perform client home visits and reassessments as needed.
  • Respond promptly to client and caregiver concerns and resolve issues professionally.
  • Maintain Changes in Condition (CIC) records and coordinate with the RN Clinical Director when appropriate.
  • Ensure care plans are followed and updated as client needs change.
  • Assist with caregiver matching based on client needs, personality, and availability.
  • Collaborate with the Scheduling Coordinator to maintain consistent coverage.
  • Document all communication and client interactions accurately in WellSky.
  • Maintain HIPAA compliance and agency standards.
  • Participate in rotating back up on-call responsibilities.
  • Perform other duties assigned by the Director of Operations.

Qualifications

  • Minimum 2 years of experience in home care, healthcare, case management, staffing, customer service, or a related field preferred.
  • High school diploma.
  • Excellent communication and customer service skills.
  • Strong organizational and time management abilities.
  • Ability to multitask and prioritize in a fast-paced office.
  • Professional phone etiquette.
  • Proficient with Microsoft Office and computer systems.
  • Experience with WellSky or other home care software is a plus.
  • Reliable transportation and a valid driver's license.
