About the role
The Care Coordinator RN is responsible for assessing patients' transitional care needs, coordinating across the care continuum, and engaging with patients and families to ensure appropriate care planning and discharge. The role aims to enhance patient outcomes through effective care coordination and discharge management.
Responsibilities
- Conducting psychosocial and functional assessments
- Collaborating with interdisciplinary teams
- Managing discharge planning
- Facilitating post-acute referrals
- Identifying and resolving barriers to timely discharge
- Promoting efficient resource utilization
- Ensuring comprehensive documentation of care activities
- Professional development, mentoring, and adherence to organizational policies
Qualifications
- Associate's or Bachelor's Nursing degree
- RN licensure (single or multi-state)
- BLS certification
- At least 1 year of nursing experience in acute care
