Role Overview The Care Coordinator RN (CC RN) is responsible for assessing patient needs, coordinating care across the continuum, and facilitating smooth transitions from hospital to next care level. Key Responsibilities Assess clinical, psychosocial, and social determinants of health to develop individualized discharge and care plans.Collaborate with patients, families, physicians, and interdisciplinary teams to ensure timely care progression and discharge.Manage discharge planning, identify barriers, and facilitate post-acute referrals.Coordinate communication on patient progress, resolve delays, and ensure documentation accuracy.Participate in interdisciplinary rounds and support patient education. Qualifications & Skills Associate's or Bachelor's Nursing degree; RN license (single or multi-state compact); BLS certification.Minimum of 1 year nursing experience in an acute care setting.Strong communication, assessment, organizational, and problem-solving skills.Knowledge of healthcare regulations, community resources, and care transition processes.Ability to work collaboratively, handle stress, and adapt in a fast-paced environment. The role emphasizes compassionate, efficient, and patient-centered care coordination to improve health outcomes and streamline discharge processes.
