Care Coordinator - RN

About the role

The Care Coordinator RN (CC RN) assesses patients' transitional care needs, coordinates across the care continuum, and engages patients and families to ensure care goals are met.

Responsibilities

• Conduct psychosocial and functional assessments
• Develop individualized discharge plans
• Facilitate interdisciplinary care conferences
• Collaborate with physicians and care teams to promote timely discharge and appropriate care progression

Qualifications

• RN license (single state or multi-state compact)
• BLS certification
• At least 1 year of acute care nursing experience
• Strong communication, assessment, organizational, and problem-solving skills
• Knowledge of healthcare regulations and community resources
