Care Coordinator - RN

Role: The Care Coordinator RN (CC RN) assesses patients' transitional care needs, coordinates across the care continuum, and engages patients and families to ensure care goals are met. Responsibilities: Conduct psychosocial and functional assessments, develop individualized discharge plans, facilitate interdisciplinary care conferences, and collaborate with physicians and care teams to promote timely discharge and appropriate care progression. Qualifications: Requires an RN license (single state or multi-state compact), BLS certification, at least 1 year of acute care nursing experience, and strong communication, assessment, organizational, and problem-solving skills. Knowledge of healthcare regulations and community resources is essential. Key Duties: Manage discharge planning, address barriers to care, document interactions accurately, and support professional development.
