About the role
Care Coordination RNs at High Plains Mental Health Center play a critical role in ensuring patients receive safe, continuous, and well-coordinated care during transitions from inpatient psychiatric units, hospitals, and residential programs back into community care.
Responsibilities
- Work closely with providers, nurses, and community partners to keep treatment plans aligned and effective.
- Review medications and coordinate follow-up care to improve patient safety.
- Educate and support patients so they understand their care plan and feel confident moving forward.
- Build relationships across healthcare systems that strengthen mental health care in our region.
Requirements
- Current Registered Nurse (RN) licensure from the Kansas State Board of Nursing.
- Bachelor of Science in Nursing (BSN) preferred.
- Experience in psychiatric/mental health nursing, or physician office or outpatient practice preferred.
- Experience in care coordination or case management preferred.
- Strong interpersonal and collaboration skills when working with multidisciplinary teams and external providers.
- Effective communication skills when discussing treatment plans and care transitions with patients and families.
- Demonstrated proficiency with technology including computers, electronic records, and common office equipment.
- A commitment to continuous learning and quality improvement, particularly in mental health care and clinical processes.
Qualifications
