Job Summary

The RN Care Coordinator oversees the progression of care and discharge planning for identified patients requiring these services. The role emphasizes care coordination, communication, and collaboration with various departments to achieve optimal outcomes.

Responsibilities

• Oversee the progression of care and discharge planning for identified patients.
• Communicate and collaborate with utilization management, nursing, physicians, ancillary departments, insurers, and post-acute service providers to progress care toward optimal outcomes.
• Advocate for the patient and family by identifying, valuing, and addressing patient choice, spiritual needs, cultural, language, and socioeconomic barriers to care transitions.
• Strive to enhance the patient experience.

Requirements

• Graduate of an Accredited School of Nursing.
• Nursing Diploma or Associates Degree in Nursing required, BSN preferred.
• RN License in Applicable State of Employment required.
• Minimum two (2) years of acute hospital clinical experience or a Master's degree in Case Management or Nursing field in lieu of acute experience.
• At least five years of total nursing experience required.
• Certified Case Manager (CCM), Accredited Case Manager (ACM-RN), or UM Certification preferred.
• Experience working with EMR preferred.
• Working knowledge of regulatory requirements and accreditation standards strongly preferred.
• BLS from the American Heart Association required within three months of hire if located within hospital.

Physical Requirements

• Medium Work - Exert up to 50lbs force occasionally, and/or up to 20lbs frequently, and/or up to 10lbs constantly.

Pay Range

$34.14 - $61.23 /hour*
