Care Coordinator RN

Role Overview: The Care Coordinator RN (CC RN) assesses patient needs, manages transitional care, and collaborates with the interdisciplinary team to ensure effective discharge planning and care progression. Key Responsibilities Conduct psychosocial, functional, and medical assessments to develop individualized care and discharge plans.Coordinate with physicians, care teams, patients, and families to facilitate smooth transitions and address barriers to discharge.Manage all aspects of discharge planning, including post-acute referrals and resource provision.Identify and resolve delays in care or treatment, proactively addressing obstacles to timely discharge.Document assessments, care plans, interactions, and services accurately and timely in medical records.Participate in interdisciplinary rounds and care conferences to support care advancement and resource utilization.Support professional development, serve as a mentor if appropriate, and ensure compliance with organizational policies. Qualifications & Skills Licensed RN (single or multi-state compact) with BLS certification.Minimum of 1 year of nursing experience in an acute care setting.Strong communication, organizational, assessment, and problem-solving skills.Knowledge of healthcare regulations and community resources.Ability to work collaboratively, handle stress, and adapt in a fast-paced environment.
