Care Coordinator RN

About the Role

The Care Coordinator RN (CC RN) assesses and manages patients' transitional care needs, coordinating across care settings to ensure smooth discharge and ongoing support.

Responsibilities

• Conduct psychosocial and functional assessments.
• Develop individualized discharge plans.
• Facilitate interdisciplinary rounds.
• Collaborate with physicians and care teams to optimize care progression.
• Manage discharge processes and identify barriers.
• Coordinate post-acute referrals.
• Document care activities accurately and track avoidable days.
• Support professional development.
• Ensure timely care transitions and promote resource utilization.
• Advocate for patient needs and work proactively to resolve delays and barriers to discharge.

Requirements

• Active RN license (single or multi-state).
• BLS certification.
• At least 1 year of acute care nursing experience.
• Excellent communication, organizational, and problem-solving skills.
• Knowledge of healthcare regulations and community resources.
