Care Coordinator RN
About the Role
As a Case Manager, you will be a pivotal healthcare professional, delivering compassionate, high-quality care that impacts patients' well-being and recovery. The role supports the physician and interdisciplinary team in facilitating patient care, enhancing clinical outcomes and patient satisfaction while managing the cost of care. It integrates and coordinates utilization management, care facilitation, and discharge planning functions, collaborating with the Care Coordination team, including RNCC and Social Worker.
Responsibilities
- Manage timely post-acute care referrals to assist with length of stay management and mitigation of denials.
- Communicate and collaborate with interdisciplinary teams, patients, and family to facilitate care progression.
- Ensure post-acute follow-up appointments for identified patients to assist with readmission prevention and continuity of care.
- Deliver required patient notifications and letters, and obtain patient or family signatures on required documentation to meet regulatory compliance.
- Manage transportation referrals as requested.
Requirements
- Graduate of an accredited Nursing program.
- RN License valid in the state of Nebraska.
- Basic Life Support certification from the American Heart Association.
Three to five years of clinical experience in the assigned clinical practice area is preferred.

Pay
$32.70 - $48.65 per hour.
