Care Coordinator RN

Role: The Care Coordinator RN (CC RN) assesses transitional care needs, coordinates care across the continuum, and engages with patients and families to ensure comprehensive care and successful discharge planning. Responsibilities: Conducts psychosocial and functional assessments, collaborates with care teams, manages discharge planning, facilitates communication regarding care progression, and resolves barriers to timely discharge. Documents all interactions accurately, supports post-acute referrals, and participates in interdisciplinary rounds. Ensures compliance with policies and contributes to professional development. Qualifications & Skills: Requires an Associate's or Bachelor's Nursing degree, RN licensure (single or multi-state), and BLS certification. Minimum of 1 year acute care nursing experience. Must have excellent communication, organizational, and problem-solving skills, with the ability to work under stress and in a fast-paced environment. Knowledge of healthcare regulations and community resources is essential. Key Duties: Assess patient needs, develop individualized discharge plans, facilitate interdisciplinary collaboration, identify and address delays, and ensure efficient care progression. The role emphasizes quality documentation, proactive barrier resolution, and patient-centered care to promote optimal health outcomes.
