Care Coordinator II
About the role
This position provides targeted support services to children and families experiencing emotional and behavioral challenges.
Responsibilities
- To provide targeted support services (behavior management, daily living skills, social skills); linkage with community supports; referral to local needed services; advocacy; service coordination; and follow-up on behalf of client and their family.
- To manage a crisis involving a client, including a threat of suicide or harm to others.
- To lead non-diagnostic interviews with the client and his/her family to assess service needs.
- To provide services to clients in their homes, communities, and schools (if applicable) while incorporating the entire family and supports in the treatment program, with a focus on their strengths as well as the client’s age, developmental level, and cultural heritage.
- To maintain close communication with the supervisor and other clinical and administrative staffs in order to assure the highest quality of services are delivered to the client.
- To coordinate services between agencies and develop cooperative working relationships with agencies serving children and families.
- To provide outreach services as necessary to clients when they appear to disengage from the service.
- To provide basic social skills training in such areas as: anger management, feelings identification, positive interpersonal communication, impulse/self-control, healthy relationships, and other related coping skills, using a best-practice curriculum.
- To provide crisis diversion services, including immediate intervention for individuals admitted to CRS CCBHC who are at immediate risk of admission to local hospital emergency departments, psychiatric hospital inpatient units, and jails.
- To collaboratively develop emergency safety plans/safety promises with individuals and/or families that ensure safety, match the individual’s competency and skill level, and incorporate the person’s strengths and supports.
- To follow up on clients who screen into suicide protocol after initial evaluation appointment.
- To prepare and send letters for follow up care including, but not limited to, missed appointments, expired plans, and discharged clients.
- To perform other duties as assigned.
Qualifications
- Bachelor’s/Master’s degree in a behavioral health related field or a Bachelor’s/Master’s degree in Education with at least 9 hours of credit in a behavioral health related field.
- Minimum of one year work experience with children in the mental health field or related social service field.
- The willingness and ability to work irregular hours as needed to ensure quality standards are met in service delivery.
Special Knowledge, Skills And Abilities
- Ability to establish working relationships with clinical, administrative, and support staff as well as community partners to develop professionalism and coordination within the team.
- Good working knowledge of community social service resources, basic crisis management skills, behavioral health resources, ability to provide trauma-informed care and a commitment to the Sanctuary Model components.
- Clear and respectful communication with staff, clients, and the community-in person, through written correspondence and on the telephone.
- Demonstration of professional and ethical behavior as well as the ability to maintain confidentiality guidelines for interactions regarding client information.
- Ability to work well under pressure, meeting deadlines with efficiency and consistency while maintaining a balance between good physical and mental health and the job requirements.
- Certification in mental health services or related field may be required in order to provide services, i.e. Case Management Certification, Wellness Coach Certification.
- Basic computer knowledge and ability to use Microsoft software such as Outlook, Excel, and Word.
- Understanding and promotion of Trauma-Informed Care practices.
- Practice and promotion of the Sanctuary Model’s seven commitments of non-violence, emotional intelligence, social learning, open communication, social responsibility, democracy, and growth/change.

Physical Requirements
Must be able to move from location to location, bending, stooping, lifting 25 pounds, communicate clearly, prepare written communications, present information to other employees, process complex information within established procedures/programs, enter data into various programs.

Hours
Irregular hours including some evening and/or weekend work may be required as needed to ensure quality standards are met in service delivery. This position has a weekly productivity requirement of 22.5 billable services each week. Documentation must be entered into the EHR within 48 hours of providing the service per agency standards.
