About the role
Easterseals NH is excited to announce a new Care Coordinator II role to support our Veterans!
Responsibilities
- Receive referrals of service members, veterans, and their families from ESNH Intake Staff.
- Respond promptly to new referrals and calls/requests from existing clients – within 48 hours (2 business days) maximum and as quickly as possible for emergency/urgent situations.
- Complete comprehensive and timely intake/assessment/care-planning processes with service members, veterans and families in accordance with program/contract guidelines. Submit timely documentation upon completion.
- Assess and care planning should be ongoing; provide updated assessments/care plan goals as directed by program.
- In conjunction with specific job role, develop/share expertise and function as team expert related to specialty areas such as aging veterans, veteran housing, veterans experiencing SUDs, etc.
- Provide support / resources and suicide prevention/intervention as needed.
- Document all sessions, family contact, and all other relevant contacts as it relates to the client.
- Serve as “one stop” resource for coordinating services – assist in accessing services provided by the military, civilian social service system, VA, Veterans Count, and other community resources.
- Work in collaboration with other service providers to maximize use of existing resources, facilitate prompt/comprehensive services, and minimize red tape and stigma for service members, veterans and their families.
- Access ancillary service providers and pre-existing programs whenever possible; provide services directly when no other resources are available.
- Participate in data collection/analysis activities to ensure effective program evaluation.
- Attend monthly Case Review and Core Team meetings.
- Attend Military Events (evenings, weekends) on as-needed basis.
- Aid in securing client testimonials for “Consumer Moments” and other presentations.
Qualifications
- Master’s degree in human services or related field (counseling, social work, social welfare, social and behavioral sciences, education, human development, mental health and human services, psychology, psychology/educational psychology, rehabilitation services, and sociology) with two (2)+ years relevant experience, OR
- Bachelor’s degree with five (5)+ years related experience as specified under Essential Responsibilities
- Understanding of military structure and culture preferred
- Valid driver’s license and reliable transportation

Pay
$26.11 - $33.64 per hour
Schedule
Full time 40 hours a week Monday-Friday. With travel throughout New Hampshire
