About the role
Sea Mar Community Health Centers is a Federally Qualified Health Center (FQHC) founded in 1978, serving diverse communities, with a focus on Latinos in Washington State. Our network includes over 90 medical, dental, and behavioral health clinics and various nutritional, social, and educational services.
Position Summary
A full-time Care Coordinator position is available at our Lacey Medical Clinic. The Care Coordinator works as part of a clinical care team, enhancing quality and patient-centered care by assessing gaps in care for patients with chronic conditions and/or mental health needs, and creating plans during daily huddles. They assist with medication management, insurance access, and preventive health needs, and use Motivational Interviewing skills. They also facilitate team meetings and coordinate care transitions.
Duties and Responsibilities
- Participate in morning huddles to anticipate patient needs
- Work with the care team to identify and resolve care gaps using process improvement strategies
- Provide brief interventions at point of care to assist with chronic illness management, address social needs, and link patients to behavioral health
- Advocate for patient services with community, social service, and medical providers
- Track patient adherence with plan of care in electronic or paper charts and communicate outcomes to primary care providers
- Organize monthly Health Home meetings and facilitate the meeting
- Collaborate with clinical care team to improve Patient-Centered Medical Home processes and provide documentation demonstrating performance
- Generate reports for care teams to identify areas of improvement and monitor quality measures
Qualifications and/or Experience
- Ability to work independently, prioritize workload, and meet deadlines

- Knowledge of medical terminology and/or behavioral health topics
- Strong decision-making and prioritization skills
- Respectful, professional, and empathetic communication skills

- Strong patient engagement and interpersonal skills
- Knowledge of the Patient-Centered Medical Home Model and motivational interviewing skills a plus
- Knowledge of evidence-based standards of care for chronic conditions and behavioral health issues
- Proficient in Microsoft Word, Excel, PowerPoint, and Outlook
- Ability to utilize and document relevant patient information in the Electronic Health Record
- Knowledge of community resources
- Ability to work in a fast-paced community health care setting
- Ability to think analytically and problem-solve in a multidisciplinary team and independently
- Effective communication and customer service skills
- Understanding and implementation of process improvement activities
- Knowledge of NCQA (National Committee for Quality Assurance) Certification a plus
- Valid WA State Driver’s License and proof of liability insurance
