About the role
The purpose of the Care Coordinator-Dementia is to provide assessment, coordination, and execution of the nursing care needs of individuals primarily with dementia living both long and short term at Pleasant View Nursing Home and the O’Rourke Dementia Stabilization Unit.
Responsibilities
- Receive a report from the nurse on the previous shift, listen to updates, changes, and new information.
- Present a comprehensive report to the nursing assistants.
- Confirm understanding of assignments and requests as well as the time that these are due to be completed.
- Review the schedule for therapy, the beauty shop, programs for the day and outside appointments.
- Conduct rounds on the unit upon arrival; monitoring the appearance of the unit, the appearance of the residents.
- Communicate observations to the appropriate staff for follow through and attention.
- Cookordinate the duties and responsibilities for the day with the RCC and/or the RN Supervisor.
- Monitor the duties of the nursing assistants for completeness and in conjunction with the plan of care.
- Record resident information using the electronic chart or the clinical record.
- Responsible for the care and service to the residents in coordination with the RCC and under the direction of the DON.
- Responsible to fulfill the duties of a staff nurse.
- Complete assessment of the resident upon admission, quarterly and upon change of condition.
- Complete accident/incident skin injury reports as necessary reviewing them for completeness.
- Complete the investigation of the root cause.
- Monitor the follow up review of care plans for effective and appropriate interventions.
- Communicate with Medical Providers reviewing the plan of care with updates as to improvements, changes in condition and decline in condition.
Qualifications
- Average or Bachelor’s degree in nursing with valid Wisconsin RN license.
- Two years nursing experience, or any combination of education and experience that provides equivalent knowledge, skills, and abilities.
- Dementia nursing experience preferred.
