About the role

Ennoble Care is a mobile primary care, palliative care, and hospice service provider with patients in various states including Georgia. The Care Coordinator role is based in the College Park area of Atlanta, Georgia, and works in an office setting alongside fellow Care Coordinators.

Responsibilities

  • Coordinate patient-centered care activities using Ennoble Care’s electronic medical record (EMR) system
  • Support Ennoble Care providers and clinical teams through accurate documentation, follow-up tracking, and care coordination
  • Serve as a key point of communication between providers and internal care teams regarding clinical updates, patient needs, and care coordination priorities
  • Communicate patient needs, changes in condition, and urgent concerns to the clinical team in a timely and appropriate manner
  • Aid in care transitions, referrals, and coordination with external partners, as needed
  • Collaborate closely with fellow Care Coordinators and internal care team members within an office-based environment
  • Maintain accurate, timely documentation in accordance with established workflows, documentation standards, and escalation protocols to support timely decision-making and efficient clinical operations
  • Contribute positively to team collaboration, workflow efficiency, and day-to-day office operations

Requirements

  • Legally authorized to work in the United States of America
  • Active, unrestricted clinical certification or licensure, such as: Patient Care Technician (PCT), Certified Nursing Assistant (CNA), Medical Assistant (MA)/Certified Medical Assistant (CMA), Licensed Practical Nurse (LPN), Registered Nurse (RN), or comparable credential
  • Minimum 1 year of experience in a clinical and administrative or care coordination role within a healthcare setting
  • Comfortable communicating proactively and professionally throughout the day via Microsoft Teams, email, phone, and internal systems
  • Cultivate and maintain positive, approachable working relationships while demonstrating a consistent, dependable presence
  • Well-developed organizational, time-management, and attention-to-detail skills, paired with an optimistic approach and initiative-driven mindset
  • Strong working knowledge of healthcare terminology and care coordination principles
  • Proficiency with Microsoft 365, including Outlook, Word, and Excel
  • Ability to work full-time, Monday through Friday, 8:30 a.m. to 5:00 p.m. EST, in an office-based setting in the College Park area of Atlanta, Georgia
  • No weekends
  • No on-call
  • Daytime hours
  • Seven (7) company-paid holidays annually
  • Ability to pass a background check
