About the role

The Care Coordinator plays a crucial role in supporting patients, families, and healthcare providers by serving as a resource contact, providing education, and ensuring effective communication throughout the healthcare process.

Responsibilities

  • Serves as a resource contact, education source to patients, families, providers, and/or staff
  • Telephonic follow-up with members, including post-hospital discharge and chronic condition management
  • Completes assigned tasks related to Care Coordination
  • Generates tasks to the assigned RN Case Manager
  • Directs patients/families to appropriate resources, including community resources
  • Supports the attainment of members' goals, including member education, access to resources, and follow-up care arrangements
  • Maintains quality service by adhering to organization standards
  • Aids in data entry, faxes, filing, and other clerical duties
  • Assists with handling inbound/outbound calls and gathering information from licensed staff

Requirements

  • 1+ years related work experience in a healthcare-related role
  • High school diploma or GED
  • Strong organizational and time management skills
  • Excellent verbal and written communication skills
  • Ability to communicate telephonically in a professional and efficient manner
  • Computer literacy in Microsoft Office Suite (Excel, Word, Outlook, PowerPoint)
  • Ability to multi-task in a fast-paced environment

Qualifications

  • 1+ years related work experience in a healthcare-related role
  • High school diploma or GED
