About the role
The Care Coordinator plays a crucial role in supporting patients, families, and healthcare providers by serving as a resource contact, providing education, and ensuring effective communication throughout the healthcare process.
Responsibilities
- Serves as a resource contact, education source to patients, families, providers, and/or staff
- Telephonic follow-up with members, including post-hospital discharge and chronic condition management
- Completes assigned tasks related to Care Coordination
- Generates tasks to the assigned RN Case Manager
- Directs patients/families to appropriate resources, including community resources
- Supports the attainment of members' goals, including member education, access to resources, and follow-up care arrangements
- Maintains quality service by adhering to organization standards
- Aids in data entry, faxes, filing, and other clerical duties
- Assists with handling inbound/outbound calls and gathering information from licensed staff
Requirements
- 1+ years related work experience in a healthcare-related role
- High school diploma or GED
- Strong organizational and time management skills
- Excellent verbal and written communication skills
- Ability to communicate telephonically in a professional and efficient manner
- Computer literacy in Microsoft Office Suite (Excel, Word, Outlook, PowerPoint)
- Ability to multi-task in a fast-paced environment
Qualifications
- 1+ years related work experience in a healthcare-related role
