About the role
Full-time Care Coordinator supporting children aged 0–3 with developmental delays or disabilities within a family-centered, trauma-informed Early Intervention program.
Responsibilities
- Conduct functional assessments
- Develop and implement individualized service plans
- Coordinate services and support families through education and advocacy
- Facilitate team meetings and collaborate with community providers
- Ensure accurate documentation
- Work closely with families, service providers, and internal teams to promote child development and family well-being
Qualifications
- Bachelor's degree in Social Work, Child Development, or related field
- Experience working with families with special needs preferred
- Valid driver's license and reliable transportation
- Strong organizational and communication skills
- Ability to work in varied settings, respond to crises, and manage flexible schedules
Work Environment
Community-based with travel, involving physical activity and exposure to diverse environments, including home visits and community settings.
