About the Role

Full-time Care Coordinator supporting children aged 0–3 with developmental delays or disabilities within a federally mandated Early Intervention program.

Responsibilities

  • Partner with families, service providers, and community resources to support child development
  • Conduct functional assessments and develop individualized, family-centered service plans
  • Coordinate and facilitate services
  • Provide family education, advocacy, and collaboration
  • Maintain accurate documentation
  • Lead family-centered team meetings
  • Collaborate with internal teams and community providers
  • Adapt schedules based on client needs
  • Meet productivity standards

Qualifications

  • Bachelor's degree in Social Work, Child Development, or related human services field
  • Preferred experience working with families with special needs
  • Valid driver’s license and reliable transportation
  • Strong organizational, communication, and documentation skills
  • Ability to work in various environments and handle crisis situations

Work Environment

Community-based with daily travel; moderate physical activity; potential overnight travel; exposure to diverse settings in homes and communities.
