About the Role
Full-time Care Coordinator supporting children aged 0–3 with developmental delays or disabilities within a federally mandated Early Intervention program.
Responsibilities
- Partner with families, service providers, and community resources to support child development
- Conduct functional assessments and develop individualized, family-centered service plans
- Coordinate and facilitate services
- Provide family education, advocacy, and collaboration
- Maintain accurate documentation
- Lead family-centered team meetings
- Collaborate with internal teams and community providers
- Adapt schedules based on client needs
- Meet productivity standards
Qualifications
- Bachelor's degree in Social Work, Child Development, or related human services field
- Preferred experience working with families with special needs
- Valid driver’s license and reliable transportation
- Strong organizational, communication, and documentation skills
- Ability to work in various environments and handle crisis situations
Work Environment
Community-based with daily travel; moderate physical activity; potential overnight travel; exposure to diverse settings in homes and communities.
