Care Coordinator
About the role
At Maryhurst, the Care Coordinator facilitates and supervises the treatment planning process for clients, serving as a liaison and representative for the child, family, and outside agencies. The role monitors the effectiveness and efficiency of services for Quality Assurance and interfaces with community resources such as legal, education, and medical systems. The Care Coordinator connects all services within the Maryhurst system for individual clients (therapy, medical, psychiatric, education, spiritual, etc.), assesses client/family needs, and makes referrals to necessary resources. They ensure the client is in the most appropriate, least restrictive setting and has clear goals toward permanency.
Responsibilities
- Facilitate and supervise the treatment planning process for clients.
- Serve as a liaison and representative for the child, family, and outside agencies.
- Monitor effectiveness and efficiency of services for Quality Assurance.
- Interface with community resources such as legal, education, and medical systems.
- Connect all services within the Maryhurst system for individual clients (therapy, medical, psychiatric, education, spiritual, etc.).
- Assess client/family needs and make referrals to necessary resources.
- Ensure the client is in the most appropriate, least restrictive setting with clear goals toward permanency.
Requirements
- Bachelor's degree in Human Services or related field.
- Minimum one year of experience working with family and children's services or in a case management position.
- Ability to facilitate the treatment process and collaborate with the treatment team effectively.
- Strong organizational, assessment, assertiveness, and documentation skills.

Pay
Starting salary: $44,900
