Care Coordinator

Responsibilities

• Complete Social Determinants of Health screenings and provide referrals to assist with barriers and priority needs.
• Monitor client progress towards improving access to care through follow-up with participant and service providers as needed.
• Assure the provision of education, risk reduction, and counseling to all clients regarding Mental Health and Physical Health services such as psychotropic medication management and testing or treatment for HIV/AIDS, tuberculosis, hepatitis, sexually transmitted diseases, and/or other communicable diseases or co-occurring conditions.
• Ensure entitlements, insurance, and benefits are in place.
• Cookordinate with CRPA staff to manage coordination of referrals for resources and transportation, providing warm handoffs and follow-up on outcomes with providers.
• Cookordinate with other facilities to ensure the continuity of care as clients transition between care systems.
• Conduct outreach to gather community resources and connections to address participant needs.

Qualifications

• A High School Diploma or equivalent
• Minimum of one year of related experience
• Proficient computer skills with MS Office
• Valid and clean driver's license
