Care Coordinator

Responsibilities

  • Identify seniors’ health needs
  • Build caseload by interacting with residents
  • Communicate needs to Clinic Director and/or ADO
  • Support intake process for new residents
  • Schedule evaluations and assessments
  • Auxiliary client-facing services
  • Conduct wellness classes and personal fitness training
  • Auxiliary scheduling and team member support
  • Assist with reporting and billing
  • Perform general administrative support

Qualifications

  • Certification in healthcare, business administration, or related field
  • Two years of relevant experience in a senior living setting preferred
  • Proven experience in administrative and operational roles
  • Experience in a customer-facing role
  • Understanding of wellness offerings and laws related to assisted living, independent living, and memory care
  • Exceptional communication and interpersonal skills
  • Ability to work well with others and take direction from management
  • Professional curiosity, desire to learn new things, and problem-solving skills
  • Ability to remain calm in stressful situations, be flexible, and multi-task
  • Proficiency in internet browsers and Microsoft Office programs
