About the role
Full-time Care Coordinator supporting children aged 0–3 with developmental delays or disabilities within a family-centered, trauma-informed Birth to Three Program.
Responsibilities
- Conduct functional assessments.
- Develop and implement individualized service plans.
- Coordinate service delivery.
- Support families through education and advocacy.
- Facilitate family-centered team meetings.
- Maintain accurate documentation.
- Collaborate with families, service providers, and community resources.
- Manage flexible schedules and meet productivity expectations.
Qualifications
- Bachelor’s degree in Social Work, Child Development, or related fields.
- One year of supervised experience preferred.
- Valid driver’s license and reliable transportation.
- Strong organizational and communication skills.
- Ability to work in various settings including home and community environments.
- Ability to handle crisis situations effectively.
