About the role

Full-time Care Coordinator supporting children aged 0–3 with developmental delays or disabilities within a family-centered, trauma-informed Birth to Three Program.

Responsibilities

  • Conduct functional assessments.
  • Develop and implement individualized service plans.
  • Coordinate service delivery.
  • Support families through education and advocacy.
  • Facilitate family-centered team meetings.
  • Maintain accurate documentation.
  • Collaborate with families, service providers, and community resources.
  • Manage flexible schedules and meet productivity expectations.

Qualifications

  • Bachelor’s degree in Social Work, Child Development, or related fields.
  • One year of supervised experience preferred.
  • Valid driver’s license and reliable transportation.
  • Strong organizational and communication skills.
  • Ability to work in various settings including home and community environments.
  • Ability to handle crisis situations effectively.
