About the role

Serve as the hospice expert to professionally and seamlessly coordinate services and bridge the transitions for the patient to hospice services. Develop long-term relationships at each facility to reinforce CHOT/Providence Hospice as the provider of choice.

Responsibilities

  • Respond to referral sources in a timely manner; evaluate patient desire for hospice; meet with families/patients to discuss goals and explain services of hospice.
  • Complete required paperwork with patients/families; facilitate patient discharge when needed by working with facility/hospital staff, DME, medications, hospice physicians, and other facility staff.
  • Educate facility/hospital staff and follow up on pending patients; be available to staff and patients for questions and to build relationships.
  • Communicate and provide daily feedback to admission staff on referrals.
  • Through customer service and leveraging relationships, become the “hospice of choice,” increasing hospital referrals.
  • Attend and participate in routine staff meetings in person or via media as directed.
  • Participate in and/or provide in-service training, attend various facility/hospital activities, and perform other duties as required or assigned.

Requirements

  • Four-year college degree preferred.
  • 1-3 years of healthcare marketing experience required.
  • 1 year of hospice experience required.
  • Medical knowledge strongly preferred (ability to read charts, know medications, etc.).
  • Proficient using PC including Microsoft Office.
