The Care Coordinator role assists families and staff with coordinating around their schedule and case starts. This role bridges the gap between client and employee scheduling/staffing and requires a strong commitment to delivering outstanding customer service to both families and staff. This role reports directly to the Assistant Regional Clinical Director.

Responsibilities

  • Reach out to families once their assessment is completed to confirm availability and discuss next steps in the process, including weekly check-ins, even with no new updates, to ensure that the family and staff know they are valued.
  • Reach out to RBTs/BTs once they are hired to discuss caseloads and expectations.
  • Accurate data entry and communication recorded in EHR and other software.
  • Schedule start dates for families, coordinate closely with clinical team, families, and employees.
  • Coordinate restaff dates with families as needed.
  • Manage client and staff schedules in Central Reach.
  • Conduct monthly check-ins with families and RBTs.
  • Document conversations and follow-ups as needed.
  • Issue corrective action as needed to RBTs for attendance, unconverted session notes, accurate data collection, etc., and document in Paylocity.
  • Exercise discretion to schedule clients appropriately, maximizing their authorization.
  • Audit all parts of EHR for accuracy and completeness.
  • Evaluate staffing needs and communicate with the team.
  • Attend team meetings and communicate current needs.
  • Identify scheduling and staffing adjustments necessary and address accordingly based on client satisfaction feedback received.
  • Engage in community outreach efforts, including building relationships with local daycares and other identified partners, to support continued growth.
  • Other duties as assigned.

Requirements

  • High School diploma or GED; higher education preferred (associate or bachelor’s degree).
  • Detail-oriented and organized.
  • Excellent verbal and written communication skills.
  • Proficient with Microsoft Office Suite or related software.
  • 2 years of experience with intake/care coordination OR 2 years in an ABA OR Customer Service role.

Schedule

This is a full-time, on-site role at our Voorhees Center, supporting families in the surrounding area. Core hours are Monday–Friday, 8:30 AM–5:00 PM.

Pay

The expected salary range for this role is $55,000.00 – $63,000.00 per year. Final compensation will be based on factors such as skills, education, experience, certifications, market conditions, and internal equity.
