About the role
The role of Care Coordinator involves supporting children aged 0–3 with developmental delays or disabilities and their families through a federally mandated Birth to Three Program.
Responsibilities
- Conduct functional assessments
- Develop and implement individualized, family-centered service plans
- Coordinate services
- Facilitate team meetings
- Partner with families, service providers, and community resources to promote child development
- Provide education, advocacy, and support
Qualifications
- Bachelor’s degree in Social Work, Child Development, or related fields
- Preferred experience working with families with special needs
