About the role
We are seeking a Care Coordinator to support children with developmental, physical, or emotional disabilities through the Children's Long Term Support (CLTS) Waiver Program.
Responsibilities
- Conduct functional assessments and develop individualized service plans with families and providers
- Authorize and coordinate services to ensure children’s safety and well-being
- Facilitate family-centered team meetings and maintain accurate documentation
- Collaborate with service teams and participate in staff training
- Promote client independence using trauma-informed, evidence-based practices
- Testify at legal proceedings when required
Qualifications
- Bachelor’s degree in a health or human services field
- At least 1 year of experience working with children in the target population
- Strong communication, organization, and documentation skills
- Ability to use electronic health records and computer applications
- Valid driver’s license and reliable transportation
- Bilingual fluency in Spanish is preferred but not required
