Care Coordinator

About the role

We are seeking a Care Coordinator to support children with developmental, physical, or emotional disabilities through the Children's Long Term Support (CLTS) Waiver Program.

Responsibilities

  • Conduct functional assessments and develop individualized service plans with families and providers
  • Authorize and coordinate services to ensure children’s safety and well-being
  • Facilitate family-centered team meetings and maintain accurate documentation
  • Collaborate with service teams and participate in staff training
  • Promote client independence using trauma-informed, evidence-based practices
  • Testify at legal proceedings when required

Qualifications

  • Bachelor’s degree in a health or human services field
  • At least 1 year of experience working with children in the target population
  • Strong communication, organization, and documentation skills
  • Ability to use electronic health records and computer applications
  • Valid driver’s license and reliable transportation
  • Bilingual fluency in Spanish is preferred but not required
