About the role
We are seeking a Care Coordinator for the Children's Long Term Support (CLTS) Waiver Program to support children with developmental, physical, or emotional disabilities.
Responsibilities
- Conducting functional assessments and developing individualized service plans
- Authorizing and coordinating services in line with plans
- Facilitating family-centered team meetings and maintaining accurate documentation
- Collaborating with service teams and supervisors to determine interventions
- Providing trauma-informed, family-centered services that promote client independence
- Participating in staff training and development
- Testifying in legal proceedings if required
Qualifications
- Bachelor's degree in a health or human services field
- Minimum of 1 year experience working with children in the target population
- Strong communication, organizational, and documentation skills
- Ability to use electronic health records and computer applications
- Valid driver's license and reliable transportation
- Preference for bilingual fluency in Spanish
Additional Details
- Community-based role requiring daily travel and occasional overnight stays
