Care Coordinator

About the role

We are seeking a Care Coordinator for the Children's Long Term Support (CLTS) Waiver Program to support children with developmental, physical, or emotional disabilities.

Responsibilities

• Conducting functional assessments and developing individualized service plans
• Authorizing and coordinating services in line with plans
• Facilitating family-centered team meetings and maintaining accurate documentation
• Collaborating with service teams and supervisors to determine interventions
• Providing trauma-informed, family-centered services that promote client independence
• Participating in staff training and development
• Testifying in legal proceedings if required

Qualifications

• Bachelor's degree in a health or human services field
• Minimum of 1 year experience working with children in the target population
• Strong communication, organizational, and documentation skills
• Ability to use electronic health records and computer applications
• Valid driver's license and reliable transportation
• Preference for bilingual fluency in Spanish

Additional Details

• Community-based role requiring daily travel and occasional overnight stays
• Work involves responding to crisis situations and outdoor conditions
• Opportunities for remote work two days per week
