About the role
Care Coordinator for a Birth to Three Program supporting children aged 0–3 with developmental delays or disabilities and their families.
Responsibilities
- Conduct functional assessments
- Develop and implement family-centered service plans
- Coordinate services
- Support families through education and advocacy
- Facilitate team meetings
- Maintain accurate documentation
- Collaborate with internal teams and community providers
- Manage a flexible schedule and meet productivity expectations
Qualifications
- Bachelor’s degree in Social Work, Child Development, or related fields
- One year of supervised experience preferred
- Valid driver’s license and reliable transportation
- Strong organizational and communication skills
- Ability to work in diverse settings and respond to crises
Work Environment & Skills
Community-based role involving travel, physical activity, and home/community visits. Must be adaptable, organized, and capable of working in home, office, and community settings.
