About the role
The Care Coordinator will coordinate care between persons receiving services and their service providers within the Certified Community Behavioral Health Clinic (CCBHC) and in the community. The role aims to provide a holistic and person-centered approach to care, including meeting individuals in the community to bridge any barriers they might face in accessing their services.
Responsibilities
- Coordinate care between persons receiving services and their service providers within the CCBHC and in the community.
- Provide a holistic and person-centered approach to care, including meeting individuals in the community to bridge any barriers they might face in accessing their services.
- Be personable and professional with a wide range of people from different backgrounds.
- Accurately record data related to care coordination.
- Assist individuals and their families with other services as directed by their treatment plans.
Requirements
- High school diploma or equivalent.
- Experience in behavioral health or related field preferred.
- Strong communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Proficiency in Microsoft Office Suite.
Qualifications
- Valid Kansas driver’s license and reliable transportation.
- Background check required.

Pay
