POSITION SUMMARY
Serve as the hospice expert to professionally and seamlessly coordinate services and bridge the transitions for the patient to hospice services; develop long-term relationships at each facility to reinforce CHOT/Providence Hospice as the provider of choice.
ESSENTIAL FUNCTIONS
- Respond to referral sources in a timely manner;
- Evaluate patient desire for hospice;
- Meet with families/patients to discuss goals and explain services of hospice.
- Complete required paperwork with patients/families;
- Facilitate patient discharge when needed by working with facility/hospital staff, DME, medications, hospice physicians, and other facility staff.
- Educate facility/hospital staff and follow up on pending patients;
- Be available to staff and patients for questions and to build relationships.
- Communicate and provide daily feedback to admission staff on referrals.
- Through customer service and leveraging relationships, become the “hospice of choice,” whereby increasing hospital referrals.
QUALIFICATIONS
- Four-year college degree preferred.
- 1-3 years of healthcare marketing experience required;
- 1 year of hospice experience required.
- Medical knowledge strongly preferred (ability to read charts, know medications, etc.).
- Required to be proficient using PC including Microsoft office.
