About the role
The Hampton-Newport News Community Services Board seeks a Care Coordinator to support individuals experiencing behavioral health crises at the Regional Crisis Stabilization Center (RCSC).
Responsibilities
- Coordinate referrals and transitions of care
- Serve as a liaison between RCSC and community programs
- Support discharge planning and aftercare coordination
- Complete case management activities and accurate electronic record documentation
- Collaborate with internal/external providers
- Review pre-screening evaluations for appropriateness
- Develop client service plans
- Provide direct crisis intervention support
- Maintain confidentiality and advocacy in accordance with policy
Requirements
- Master's Degree in Human Services
- One (1) year experience providing service collaboration and coordination, psychoeducation, and behavioral health skills trainings to the client population assigned
- Must be registered with the Board of Counseling to practice as a QMHP, or register as a QMHP - Trainee within 30 days of employment
