Essential Duties and Responsibilities

  • Maintains utmost level of confidentiality at all times and adheres to hospital policies and procedures.
  • Demonstrates business practices and personal actions that are ethical and adhere to corporate compliance and integrity guidelines.
  • Possesses strong computer skills for using Word, Excel, Power Point, Access, E-mail, and Internet.
  • Possesses excellent customer services skills.
  • Available to work a flexible schedule i.e., daytime, evening, and / or weekend shifts as needed.
  • Identifies potentially dissatisfied customers and directs inquiries to the appropriate staff for follow-up and resolution.
  • Is knowledgeable about resources within the hospital and the community.
  • Cross trained in multiple areas within the department for seamless care.
  • Functions as an advisor to the Director and Senior Director to assist in the development of quality patient care services and the establishment of policies and procedures.
  • Basic Needs Assessment, Community Resourcing, and Discharge Planning
    • Aids families in community resourcing and the coordination of meal, lodging, and or transportation needs.
    • Knowledgeable about resources (i.e.: housing, shelters, nearby hotels, meal programs, MCD Transportation , meal and lodging criteria, and other community supports).
    • Aids in updating community resources as needed / requested.
    • Provides basic needs assessment and choice verification in order to address immediate discharge planning needs related to simple Durable Medical Equipment needs (wheelchair, walker, nebulizer).
    • Secures required orders / prescriptions based on Insurance Provider, initiates referrals to DME vendors, and coordinate receipt of equipment.
    • Acts as liaison between patients and/or families, clinical staff, and various community agencies and/or hospital departments for service connection.
    • Shares outcome of plan with multidisciplinary team to ensure a safe discharge and prevent delay in discharge home.
    • Completes documentation in medical record for services provided in accordance with established guidelines.
    • Attends training, conferences, and in-services as appropriate for professional development.
    • Participates in departmental meetings and hospital committees / task forces as assigned.
    • Aids in the coordination of special projects and other duties as assigned.

When Assigned to The Family Connection

  • Aids in the management of the Guest Rooms that may include: daily census verification, assignment of rooms, and tracking of room usage, bi-monthly room inventory.
  • Prepares, issues, and confirms orders for supplies and work orders for engineering and housekeeping, and completes timely room report for monthly billing for all payors.
  • Aids in the coordination and management of services provided in the Family Connection.
  • Restocking of Driscoll and Community resources.
  • Famil Pantry monitoring to include inventory, sending requests for restocking, and restocking of supplies when received.
  • Computer monitoring to ensure appropriate usage.
  • Provides Cottage Shop vouchers, bus vouchers, and taxi vouchers to families per needs assessment.
  • Disperses department petty cash as requested by Social Worker, manages daily log of dispersed funds, and coordinates reallocation of funds for accounting.
  • Aids in the generation of weekly, monthly, and as needed activity and statistical reports.
