Care Coordination Nurse
About the role
The Care Coordination Nurse provides assessment, planning, facilitation and advocacy for optimal clinical and financial outcomes. In collaboration with the attending practitioner, the family and other members of the health care team, coordinates the individualized plan of care and communicates the plan to appropriate stakeholders. The Care Coordination Nurse assists with appropriate use of resources both within the facility and external to the facility. Communication and collaboration may include coordinating admissions with referring facilities and providers, utilization review, discharge planning, transportation and care coordination. The Care Coordination Nurse ensures that high quality care is provided as efficiently and cost effectively as possible.
Responsibilities
- Provides assessment, planning, facilitation and advocacy for optimal clinical and financial outcomes.
- Collaborates with the attending practitioner, the family and other members of the health care team to coordinate the individualized plan of care.
- Communicates the plan to appropriate stakeholders including but not limited to referring facilities and providers, utilization review, discharge planning, transportation and care coordination.
- Affords assistance in the appropriate use of resources both within the facility and external to the facility.
- Assists with inpatient discharge planning tasks and possible outpatient clinic coverage up to two days/month.

- Working knowledge of CMS and other review agency standards.

- Possesses ability to effectively collaborate and delegate tasks.
- Possesses ability to relate to diverse age and demographic backgrounds.
- Computer literate and working knowledge of software applications (word, excel).
- Professionally appropriate analytical and organizational skills required.
Qualifications
- Registered Nurse AL Single State or any of the following equivalent qualifications below:
- - Registered Nurse AL NLC Multi State
- - Registered Nurse Non-AL NLC Multi State
- Basic Life Support
