Care Coordination Nurse

About the role

The Care Coordinator RN (CC RN) assesses patients' transitional care needs, coordinates across the continuum, and engages with patients and families to ensure care objectives are met.

Responsibilities

• Conduct psychosocial and functional assessments
• Develop discharge plans
• Facilitate interdisciplinary communication
• Manage resource utilization to promote timely discharges
• Collaborate with physicians, care teams, and social services to identify and resolve barriers
• Ensure documentation accuracy
• Support patient education
• Perform proactive care planning and discharge management
• Work as part of a team to facilitate efficient patient transitions and promote quality care outcomes

Requirements

• Associate's or Bachelor's degree in Nursing
• RN licensure (single-state or multi-state)
• BLS certification
• At least one year of acute care nursing experience
