Care Coordination Asst

About the role

The Care Coordination Assistant will support safe, therapeutic patient care processes from newborn to geriatric for a designated time frame. You will directly contribute to these processes while serving as the primary receptionist and secretary.

Responsibilities

  • Collaborates with and supports the Care Coordination team, including, but not limited to the RNCC and Social Worker.
  • Manage timely post-acute care referrals, to assist with length of stay management and mitigation of denials.
  • Communicate and collaborate with interdisciplinary teams, patients, and family to facilitate care progression.
  • Ensure post-acute follow-up appointments for identified patients to assist with readmission prevention and ensuring continuity of care.
  • Deliver required patient notifications and letters, and obtain patient or family signatures on required documentation as needed to meet regulatory compliance.
  • Manage transportation referrals as requested.

Requirements

  • Associates Other Associate (two-year) Degree in Business/Health-Related field, upon hire or High School Graduate General Studies, upon hire or High School GED General Studies, upon hire and Preferred: Bachelors Other or BSW, upon hire and Experience supporting clinicians in demanding and fast paced patient care environments.
  • Licensed Practical Nurse: WA, upon hire or Certified Clinical Medical Assistant, upon hire or Certified Medical Assistant AAMA, upon hire or Certified Medical Assistant NHA, upon hire or National Certified Medical Assistant NCCT, upon hire or National Registered/Certified Medical Assistant NAHP, upon hire or Registered Medical Assistant AMT, upon hire or Registered Medical Assistant, upon hire or Cert Med Asst - MMCI, upon hire or Certified Nursing Assistant: WA, upon hire or Emergency Medical Technician: WA, upon hire

Qualifications

  • Strong organizational and communication skills.
  • A proactive approach to managing day-to-day operations.
  • A commitment to providing safe, therapeutic care while working effectively with the entire healthcare team.

Skills

  • Excellent interpersonal and communication skills.
  • Ability to multitask and prioritize tasks effectively.
  • Proficiency in Microsoft Office Suite.
  • Knowledge of healthcare regulations and compliance standards.
