About the Role

The Nurse Practitioner examines and treats patients independently and in autonomous collaboration with other health care professionals, ensuring proper illness and injury care, disease prevention, diagnosis, treatment, and recovery. This role relies on education, experience, professional training, and judgment, with a wide degree of creativity and latitude expected. Works under minimal supervision under the direction of the collaborating physician and may lead and direct the work of others within the scope of practice.

Responsibilities

Technical Tasks

  • Perform bedside palliative care patient/family evaluation.
  • Assist with diagnosis, treatment, and symptom management of acute and chronic health conditions.
  • Order, perform, and interpret treatments within the scope of professional practice.
  • Consult program medical director as needed; comply with applicable laws and regulations regarding collaborative agreements.
  • Initiate reimbursement for services rendered.
  • Assist in the development of clinical practice guidelines/standards/protocols in support of quality palliative care.
  • Provide training and continuing education for staff on palliative care.
  • Meet monthly to provide reports and outcomes in relation to Palliative Care team goals and objectives.
  • Supervise all RNs assisting with the program.

Interpersonal Relations

  • Communicate and collaborate with physicians, nurses, and other multidisciplinary team members involved in patient care.
  • Act as a patient advocate by constantly monitoring, tracking, assessing, and evaluating the quality of care.
  • Promote communication with referring physicians and the entire team regarding the plan of care.
  • Promote the professional development of the team.
  • Consistently perform 12 organizational Service Standards focused on Values, Service, and Quality.
  • Other duties as assigned.

Requirements

  • Program development experience.
  • Current license to practice professional nursing in Louisiana.
  • Master’s degree in Nursing and/or Nurse Practitioner license.
  • Current license to practice as an Advanced Practice Nurse (APN) in Louisiana.
  • Current BLS Certification.
