About the role
The Call Center Nurse (LPN) reports to the Call Center Supervisor and is non-exempt. This position is supervised by the Call Center Supervisor and does not supervise others.
Responsibilities
- Familiarize with CFHC's program philosophies, goals, and objectives.
- Comply with CFHC's policies and procedures, as well as state and federal regulations.
- Perform duties of a nurse, including scheduling, rescheduling, or canceling medical appointments by phone, transferring calls appropriately, and working provider follow-up requests on outbound calls.
- Report pertinent information or situations impacting patient care or CFHC liability to the DON.
- Attend and participate in mandatory CFHC meetings.
- Work cooperatively with other members of the clinical team and support coworkers.
- Screen patients according to CFHC's telehealth protocols and perform any other procedures directed by the provider, Nurse Manager, or DON.
- Accurately obtain patient’s medical history and perform accurate charting in the patient’s medical record.
- Counsel patients according to the treatment plan and understand all emergency protocols.
- Accurately make, cancel, or reschedule appointments in the computer appointment system and follow-up on appointments to ensure patient compliance.
- Aid in the referral process of patients to outside services or providers and assist in follow-up on patient referrals.
- Maintain patients' concerns within the electronic medical records (EHR) and communicate with patients via various means.
- Complete all duties, responsibilities, and competencies in a manner appropriate to the age groups served.
- Perform other reasonable related business duties as assigned by the supervisor.
Qualifications
- Graduate of an accredited School of Nursing.
- Florida license and BLS certification.
- Computer literacy.
- Preferred experience in community health.
