About the role
This role focuses on building relationships with hospitals, senior communities, hospice, and local organizations to generate referrals and support families in need.
Responsibilities
- Visit healthcare and community partners (10–15 visits/week)
- Build relationships with social workers, nurses, and care teams
- Generate and follow up on referrals
- Attend networking events and host Lunch & Learns
- Track activity and results weekly
