About the role

This role focuses on building relationships with hospitals, senior communities, hospice, and local organizations to generate referrals and support families in need.

Responsibilities

  • Visit healthcare and community partners (10–15 visits/week)
  • Build relationships with social workers, nurses, and care teams
  • Generate and follow up on referrals
  • Attend networking events and host Lunch & Learns
  • Track activity and results weekly

Pay

$250 per new client start
