About The Role

We are looking for a Bridge Care Coordinator to join our Bridge Program Team. This role bridges the gap between home health and hospice services, ensuring a smooth transition for patients and providing consistent, compassionate support.

Key Responsibilities

• Conduct scheduled in-home non-clinical visits with Bridge Patients.
• Educate families about Aegis Healthcare's continuum of services.
• Connect patients to valuable external resources.
• Bridge the gap between home health and hospice by offering guidance and support to patients and families.
• Offer companionship that fosters trust and well-being for seniors.
• Foster and maintain strong, collaborative relationships with facility staff.

Qualifications

• Be at least 18 years old.
• At least 1 year in healthcare marketing (preferred).
• Possess a valid driver's license, clean driving record, and reliable automobile.
• Provide proof of current automobile insurance (MVR required).
• Have a clear negative TB test.
