Riverside Community Care is seeking a dedicated individual to join our team as a Case Manager. The ideal candidate will have strong interpersonal skills and a passion for helping others.
Responsibilities
- Assess client needs and develop care plans
- Coordinate services and resources for clients
- Maintain accurate records and reports
- Communicate regularly with clients and their families
- Participate in team meetings and training sessions
Requirements
- Bachelor's degree in Social Work, Psychology, or related field
- At least 2 years of relevant work experience
- Valid driver’s license and access to own vehicle
- Excellent communication and organizational skills
Qualifications
- Knowledge of community resources and services
- Ability to work independently and as part of a team
- Strong problem-solving and decision-making skills
Skills
- Proficient in Microsoft Office Suite
- Experience with case management software
- Strong written and verbal communication skills
