Blended Case Manager
Requirements
A qualified candidate will possess the following qualifications:
- Case Management or similar experience preferred
- High school diploma and 12 semester credit hours in sociology, social welfare, psychology, gerontology, or other social science, and 2 years' experience in public or private human services with 1 year in direct care required
- Valid Driver's license and clean driving record required
- Must be able to work Monday-Friday 8 am - 4:30pm
Responsibilities
Include Rehabilitation & Consumer Support
- Demonstrate a recovery-oriented attitude, use a consumer-driven and strength-based approach, assist with basic living needs, and ensure annual physicals and medical monitoring.
- Case Management & Service Coordination
- Act as primary case manager, conduct minimal bi-weekly support contacts, complete service/safety plans, coordinate benefits and housing, and follow through with clinical decisions.
- Consumer Tracking & Documentation
- Locate consumers when unreachable, secure past records, develop and update social histories and clinical summaries, maintain accurate service-use summaries, and complete re-authorization paperwork.
- Collaboration & Team Participation
- Coordinate services with providers, participate in treatment team meetings and service plan meetings with families and providers, act as liaison with supports, and attend all required meetings and trainings.
- Professionalism & Administrative Duties
- Maintain professionalism per policy, complete paperwork accurately and on time, and fulfill on-call duties in the assigned rotation.

This position is community-based and requires travel to homes and other community locations to provide services and support clients. Mileage reimbursement provided.
Earn $20.07 per hour in the full-time (40 hr ) role. *An additional $2 per hour is available with the selection of our Enhanced Pay option!
