Blended Case Manager

About the role

This position provides monitoring, linking and coordination of services to individuals with a mental health diagnosis, in Elk and Cameron Counties. Provides wellness coaching to designated consumers along with coordinating these activities with the Wellness Nurse. Ensures adherence to state and provider regulatory standards. This position also participates in an on-call schedule. This position may also provide Administrative Case Management services to individuals on their caseload as those consumers no longer require the intensity of the Blended Case Management program.

Responsibilities

• Mental health diagnosis monitoring and coordination
• Wellness coaching for designated consumers
• Coordinating wellness coaching activities with the Wellness Nurse
• Ensuring adherence to state and provider regulatory standards
• Participating in an on-call schedule
• Providing Administrative Case Management services to eligible consumers

Requirements

• Bachelor's degree with major course work in human service, education or social science field
• Registered Nurse certification
• High school diploma and 12 semester credit hours in human service, education or social science AND two years of experience in direct contact with mental health consumers

Qualifications

• Valid driver's license with clean driving history

Pay

$18.38-$21.24 per hour

Schedule

Full Time - 40 Hours
