About the role
We are hiring a Blended Case Manager to join our team at RHA Health Services. The Blended Case Management (BCM) program assists individuals with various behavioral health/medical needs by connecting them with community resources and services. This role involves advocating for clients, negotiating complex systems, and providing crisis intervention.
Responsibilities
- Access community resources for clients
- Communicate with clients weekly to update service plans
- Solve problems and address concerns for clients
- Develop and maintain a network of social supports
- Observe and report significant changes in client's health
- Collaborate with county office personnel and other service providers
- Provide 24/7 on-call crisis services
- Maintain case records and complete documentation
- Adhere to charting requirements and policies
- Assist clients in obtaining funding for services
- Participate in agency investigations
Requirements
- Bachelor’s degree in human services related field OR Associate’s Degree with 12 college credits relevant to position and 2 years’ experience in mental health/human service field
- Valid driver’s license

Pay
$20.00/hr
Sign-On Bonus
- $1000 sign-on bonus after 90-day probationary period
- $250 after 6 months of employment
- $500 after 1 year of employment
