Blended Case Manager

About the role

We are hiring a Blended Case Manager to join our team at RHA Health Services. The Blended Case Management (BCM) program assists individuals with various behavioral health/medical needs by connecting them with community resources and services. This role involves advocating for clients, negotiating complex systems, and providing crisis intervention.

Responsibilities

• Access community resources for clients
• Communicate with clients weekly to update service plans
• Solve problems and address concerns for clients
• Develop and maintain a network of social supports
• Observe and report significant changes in client's health
• Collaborate with county office personnel and other service providers
• Provide 24/7 on-call crisis services
• Maintain case records and complete documentation
• Adhere to charting requirements and policies
• Assist clients in obtaining funding for services
• Participate in agency investigations

Requirements

• Bachelor’s degree in human services related field OR Associate’s Degree with 12 college credits relevant to position and 2 years’ experience in mental health/human service field
• Valid driver’s license

Pay

$20.00/hr

Sign-On Bonus

• $1000 sign-on bonus after 90-day probationary period
• $250 after 6 months of employment
• $500 after 1 year of employment
