Blended Case Manager
About the role
CGRC’s BCM (Blended Case Management) provides support to children and families dealing with mental illness, helping them achieve recovery and independence. The role involves service coordination for individuals with severe social, emotional, and behavioral disorders, often in collaboration with a treatment team and various service systems.
Responsibilities
- Establish and maintain linkages with child-serving systems including mental health providers, Children and Youth agencies, drug and alcohol treatment programs, juvenile justice services, and the educational system.
- Arrange for necessary behavioral health services and communicate relevant information to service providers.
- Arrange for aftercare services and ensure the recommended level of care is accessed promptly.
- Identify and access available community resources for clients and families.
Requirements
The ideal candidate should be a Spanish-speaking clinician, demonstrating strong interpersonal skills and the ability to work effectively with diverse populations. Experience in case management, familiarity with child-serving systems, and a commitment to mental health advocacy are preferred.
Qualifications
- Bachelor's degree in Psychology, Social Work, or related field.
- Experience in case management or related field.
- Spanish language fluency is required.
- Valid driver’s license and reliable transportation.
Skills
- Strong communication and interpersonal skills.
- Ability to build and maintain relationships with various service providers and community organizations.
- Proficiency in using technology for case management and documentation.
