Blended Case Manager

Position Overview

The Blended Case Manager (BCM) position is responsible for developing, coordinating, and implementing services to assigned consumers as identified in their service plan. The BCM will work and assist their assigned consumers by focusing on the following life domains: behavioral and physical healthcare needs, family functioning/independent living, educational and vocational goals, and socialization. This role is based in Philadelphia, PA.

Requirements

• A bachelor's degree in psychology, social work, nursing, rehabilitation, counseling, or other related human services field
• Experience in the human services field working with children is preferred
• Must have a valid driver's license and own a reliable means of transportation
• Bi‑lingual a plus

Pay

Earn $23.50 per hour.
