About the role
The Blended Case Manager is responsible for providing comprehensive support to families in need, ensuring that they receive the necessary resources and services to thrive.
Responsibilities
- Assess family needs and develop personalized plans to meet those needs.
- Coordinate service delivery from various community partners.
- Maintain regular contact with families to monitor progress and adjust plans as needed.
- Document all interactions and outcomes in a timely manner.
Requirements
- Bachelor's degree in Social Work, Psychology, or a related field.
- At least 2 years of relevant experience in social services or related field.
- Strong communication and interpersonal skills.
- Proficiency in Microsoft Office Suite.

- Valid driver’s license and access to reliable transportation.
- Ability to work flexible hours including evenings and weekends.
