Blended Case Manager

About the role

The Blended Case Manager is responsible for providing comprehensive support to families in need, ensuring that they receive the necessary resources and services to thrive.

Responsibilities

• Assess family needs and develop personalized plans to meet those needs.
• Coordinate service delivery from various community partners.
• Maintain regular contact with families to monitor progress and adjust plans as needed.
• Document all interactions and outcomes in a timely manner.

Requirements

• Bachelor's degree in Social Work, Psychology, or a related field.
• At least 2 years of relevant experience in social services or related field.
• Strong communication and interpersonal skills.
• Proficiency in Microsoft Office Suite.

• Ability to work flexible hours including evenings and weekends.
