Bilingual Spanish Health Coach and Educator (Home Visits in San Gabriel Valley / Southeast LA, CA | Field Remote)

Responsibilities

• Contacts patients eligible for Alignment’s Care at Home program and provides wellness coaching to reduce high-risk behaviors
• Collaborates with advanced practice clinicians and/or social workers to develop an action plan
• Encourages adoption of habits conducive to a higher quality of life
• Delivers high-level patient service and increases member retention and satisfaction
• Aids patients in accessing community resources and social services to address social determinants impacting health and wellbeing
• Develops and distributes health education materials to identified patients, caregivers, and families
• Coordinates care with other Alignment team members, including health-related vendors for optimal patient care

Requirements

• Minimum 1+ year of experience as a wellness or health coach or educator
• Solid knowledge of specific diseases, lifestyle-related topics, nutrition, and exercise
• Experience with applying the stages of change or a similar behavior change model
• Excellent interpersonal skills with the ability to foster trust and respect among customers and coworkers
• Ability to work independently and remotely
• Proficient computer skills including Microsoft Office Excel, Word, and PowerPoint
• Effective written and oral communication skills
• Advanced problem-solving and organizational skills
• Flexible scheduling ability

Education and Training

• High School Diploma or GED
• Associate’s degree or two years of additional experience in lieu of education
• Bachelor’s Degree preferred

• Preferred: Senior care experience

Pay Range

$48,373.00 - $72,559.00
