About the role

We're looking for a bilingual Spanish-speaking Clinical Navigator (Health Coach) to support Medicare patients in our care programs. This role involves proactive outreach, coaching, and care coordination.

Responsibilities

  • Conduct outbound telephonic patient outreach and provide one-on-one coaching and support to patients managing chronic conditions.
  • Help patients execute personalized care plans and achieve their goals by providing education and coaching focused on behavior modification, nutrition, physical activity, and self-management strategies.
  • Conduct comprehensive assessments of patients' health status, lifestyle behaviors, nutritional habits, and readiness to change.
  • Educate patients on disease management, medication adherence, symptom recognition, and prevention strategies.
  • Maintain accurate clinical records in the Cadence platform and document patient interactions, outcomes, and follow-up plans.
  • Collaborate in real time with clinicians and patient support teams using tools such as the Cadence Platform, Google Workspace, and Slack.

Requirements

  • An active NBHWC health coaching certification.
  • At least 1 year of experience as a health coach.
  • Experience providing patient education and support to individuals managing chronic conditions.
  • Remote work readiness (stable high-speed WiFi).
  • Fluency in Spanish.
  • Experience using AI-assisted clinical tools (such as Gemini, Open Evidence, or comparable platforms).

  • Comfortable working independently in a remote, fast-paced environment.
  • Ability to navigate and troubleshoot multiple systems simultaneously.
  • Biased toward action, making clinical decisions confidently.
