About the role
We're looking for a bilingual Spanish-speaking Clinical Navigator (Health Coach) to support Medicare patients in our care programs. This role involves proactive outreach, coaching, and care coordination.
Responsibilities
- Conduct outbound telephonic patient outreach and provide one-on-one coaching and support to patients managing chronic conditions.
- Help patients execute personalized care plans and achieve their goals by providing education and coaching focused on behavior modification, nutrition, physical activity, and self-management strategies.
- Conduct comprehensive assessments of patients' health status, lifestyle behaviors, nutritional habits, and readiness to change.
- Educate patients on disease management, medication adherence, symptom recognition, and prevention strategies.
- Maintain accurate clinical records in the Cadence platform and document patient interactions, outcomes, and follow-up plans.
- Collaborate in real time with clinicians and patient support teams using tools such as the Cadence Platform, Google Workspace, and Slack.
Requirements
- An active NBHWC health coaching certification.
- At least 1 year of experience as a health coach.
- Experience providing patient education and support to individuals managing chronic conditions.
- Remote work readiness (stable high-speed WiFi).
- Fluency in Spanish.
- Experience using AI-assisted clinical tools (such as Gemini, Open Evidence, or comparable platforms).

- Ability to navigate and troubleshoot multiple systems simultaneously.
