Bilingual Spanish Health Coach and Educator (Conduct Home Visits in LA/OC, CA w/ Mileage Reimbursement)

About the role
As a Hybrid Health Coach & Educator, you will conduct home and virtual (telehealth and telephonic) visits to develop wellness coaching relationships with patients, caregivers, and families. You will actively support patients in achieving better health through education, encouragement, and collaboration. Must conduct between 6-8 home visits per day. Mileage reimbursement is provided from the time you leave home to the time you return.
Responsibilities
- Contact patients eligible for Alignment’s Care at Home program and provide wellness coaching to reduce or eliminate high-risk behaviors.
- Collaborate with advanced practice clinicians and/or social workers to develop action plans.
- Encourage adoption of habits that improve quality of life.
- Deliver high-level patient service to increase member retention and satisfaction.
- Assist patients with accessing community resources and social services to address social determinants of health.
- Develop and distribute health education materials to patients, caregivers, and families.
- Coordinate care with other Alignment team members and health-related vendors for optimal patient outcomes.
Requirements
- Minimum 1+ year of experience as a wellness or health coach or educator.
- High School Diploma or GED; Associate’s degree or two years of additional experience in lieu of education.
- Current and valid California Driver’s License and reliable transportation.
Preferred Qualifications
- Bachelor’s Degree.
- Senior care experience.
- Bilingual in English and Spanish.

Pay
Pay Range: $48,373.00 - $72,559.00. Pay may vary based on factors including market location, education, responsibilities, and experience.
Schedule
Monday - Friday, 8:00 AM - 5:00 PM Pacific Time.
